What You Need For Your Visit


Patients should request refills at the time of their appointment. If an appointment is not scheduled, please request refills to your pharmacy.


We make every attempt possible to approve refills within 2 business days. Please make sure you always have enough medication to last 1 week before your next refill.


Prior authorizations are at the discretion of your insurance company. If a prior authorization is required your pharmacy will contact us. This process may delay the time to refill.


Note: We do not refill medication requests called in after regular business hours nor on the weekends.

Note, we are not on all Medicare medical plans and therefore this may make you responsible for those costs that this plan may not have covered.

Prescription Refills


We know your time is important, so please fill out and bring the following items with you, for your visit.

  1. Your Picture ID (driver's license, government issued ID or employee issued ID with photo)
  2. Your Insurance Card (if enrolled in insurance)
  3. Please note we take most insurance, unfortunately not all
  4. Payment (we accept cash, debit card, Visa, Mastercard, Discover, and American Express)
    We are sorry we are unable to take checks
  5. Your previous immunization record
  6. A list of previous medical information (current medications, chronic illness, etc.) See our Patient Information form, Medical History form.

Laboratory Services


Our office uses Clinical Pathology Laboratories (CPL). If you or your insurance prefer another lab, please let our medical staff know prior to the collection of the specimen. If the CPL does not accept your insurance, you will be billed for your laboratory work.
